HLA? B27 positive ankylosing spondylitis and Correlation Analysis
Key words Ankylosing spondylitis HLA? B27 correlation analysis ankylosing spondylitis (ankylosing spondylits, AS) is a violation of the sacroiliac joint ﹑ chronic inflammatory disease of the spine, early diagnosis and early treatment is of great significance, but the current diagnostic criteria are too strict, many patients the disease once the diagnosis has been late. As human histocompatibility antigen (HLA) study, found that AS and HLA? B27 antigen showed a strong correlation, which for the early diagnosis of AS may be provided. January 2006 ~ October 2007, the author of 1,000 suspected cases and 1,000 healthy people of the examination and analysis of HLA? B27 positive ankylosing spondylitis and related analysis reports are summarized below. 1 Materials and methods 1.1 out-patient clinical data in patients with low back pain more than 3 months, with morning stiffness, pain reduction ﹑ after the event of suspected cases to 1000 cases of suspected group, which 648 cases male, female 352 patients, aged 18 to 46 years, mean 28 years of age. 1000 cases of normal healthy for the healthy group, male 656, female 344 cases, aged 17 to 50 years, mean 30 years old. Two groups of gender,[link widoczny dla zalogowanych], age difference was not significant, comparable. 1.2 Methods suspected group by flow cytometry analysis of serum HLA? B27, CT sacroiliac joint TLC; healthy set of checks of serum HLA? B27, found that positive line of sacroiliac joint CT. 2 results 2.1 serum HLA? B27 and the sacroiliac joint CT findings of 1000 cases of suspected group, HLA? B27 positive for 680 cases. New York, 1984, the standard reference to the radiological criteria [1], 1000 cases of sacroiliitis grade 0 in 752 cases, 20 cases of grade 1, 2 100 cases, 119 cases of grade 3, 4, 9 cases in Table 1. By New York standards [2], sacroiliitis grade 2 or a total of 228 patients with clinical diagnosis and signs of AS, in which HLA? B27 positive for 217 patients (95.2%), HLA? B27 negative in 11 cases (4.8%). 1000 cases in the healthy group, HLA? B27-positive 88 cases, HLA? B27 positive row CT examination revealed 2 cases of sacroiliitis, are one. Table 1 suspected group HLA? B27 and sacroiliitis relations cases HLA? B27n sacroiliitis 0 a 2 3 4 negative positive 68044815941158 3203045641 2.2 Correlation analysis of 1000 cases of suspected group, HLA? B27-positive 680 cases, the positive rate was 68.0%; healthy group of 1000 cases, HLA? B27-positive 88 cases, the positive rate was 8.8%. By cohort studies, relative risk RR (relative risk) calculated: RR = ad / bc (Woolf formula), a: suspected group HLA? B27 positive cases; b: suspected group HLA? B27 negative patients in the number; c: health group HLA? B27 positive cases; d: health group HLA? B27 negative patients in number. When RR> 1 for calculating the percentage of population attributable risk (population attributable risk percent, PAR%): PAR = (RR? 1) ÷ RR × a ÷ (a + b). Suspected group of HLA? B27 positive rate was significantly higher than the healthy group, RR = 22, PAR% = 64.9%, prompted HLA? B27 positive highly correlated with AS. 1000 cases of suspected cases, sacroiliitis grade 2 and above, a total of 228 patients diagnosed with AS, including HLA? B27-positive 217 cases, accounting for 95.2%, suggesting that HLA? B27 has a strong positive correlation with AS. HLA? B27 positive, with the severity of sacroiliitis, more helpful in the diagnosis of AS.
